By checking the box below, I, the legal guardian, grant permission for my child to participate in the Level-Up Brother Program, offered by The Cloud Family Foundation. I acknowledge and agree to the following terms:

Voluntary Participation

I understand that my child's participation in the Level-Up Brother Program is voluntary. I am aware that this program is designed to support the personal and educational development of participants through various activities, mentoring, and events.

Program Activities

I give permission for my child to engage in all activities related to the Level-Up Brother Program, including but not limited to workshops, mentoring sessions, educational outings, and community service projects.

Photography and Media Release

I consent to The Cloud Family Foundation using my child's likeness in any photographs, videos, or other media captured during program activities. I understand that these images may be used in promotional materials, publications, or online platforms associated with the Level-Up Brother Program, and I waive any rights to royalties or other compensation.

Liability Release

I release and hold harmless The Cloud Family Foundation, its staff, volunteers, and partners from any and all claims, demands, or causes of action related to my child's participation in the Level-Up Brother Program, including any that may arise from accidents or injuries during program activities.

Medical Treatment Authorization

In the event of an emergency, I authorize The Cloud Family Foundation and its representatives to seek and obtain necessary medical treatment for my child. I understand that I am responsible for any medical expenses incurred.

Code of Conduct

I acknowledge that my child is expected to adhere to the code of conduct established by the Level-Up Brother Program. I understand that any violation of this code may result in my child's removal from the program.
